Commitment Agreement – NC TACT - Peer Support Team

I, ____________________________ agree to serve as a volunteer member of the North Carolina Telecommunicators of APCO Critical Incident Stress Management Peer Support Team. I understand that serving, as a team member, requires the following commitment and obligations:

·         Attendance at the initial ICISF Basic/Advanced training sessions

·         Attendance at additional training sessions as required

·         Completion of any cross-training or in-house training that is identified

·         Attend team meetings as required.

·         Complete any required records or paperwork.

·         Report all CISM activities to the identified team contact.

Revocation/suspension of my membership will occur under the following circumstances, but is not limited to:

·         If I fail to maintain strict confidentiality regarding CIS debriefings held, including topics discussed and personnel involved. Any breach in confidentiality will result in immediate removal from the team and the program.

·         If I organize, or in any way attempt to organize a debriefing without contacting the CISM Program Coordinator or the appropriate identified contact person as outlined in NC TACT Standard Operating Procedures and Protocols.

·         If I organize, or in any way attempt to organize any CISM activity that is outside the defined scope of services as outlined in NC TACT CISM Standard Operating Procedures and protocols.

·         If I go to the scene or place of an incident to act on behalf of the NC TACT Program that is outside the defined scope of services as outlined in NC TACT Standard Operating Procedures and protocols.

·         If I fail to be present at an assigned briefing or activity when I have made a commitment to do so.

·         If I act contrary to the express direction of the CISM Program Manager or an involved and assigned leader or expert.

·         If I misrepresent the affairs or operations of the NC TACT Program.

·         If I am frequently absent from scheduled team meetings/training sessions.
